john-okeefe 0842ae6efa refactor: remove unnecessary type conversions and handle ignored errors across codebase
Remove redundant type conversions that Go 1.26 makes unnecessary or that
were already no-ops:

- uuid.UUID(x.Bytes) → x.Bytes (uuid.UUID is [16]byte, same as pgtype UUID Bytes)
- pgtype.UUID{Bytes: [16]byte(u), Valid: true} → pgtype.UUID{Bytes: u, Valid: true}
- (*time.Time)(&x.Time) → &x.Time
- json.RawMessage(x) → x where x is already []byte
- []byte(stringVal) → stringVal where []byte is expected
- int()/int64()/byte() casts on values already of the target type
- Decompressor(fn) → fn (type is identical)

Handle previously ignored error returns:

- collections.go: check json.Unmarshal error in GetCollection
- conversion_service.go: check fileSize.Scan() error
- app_test.go: check app.Shutdown() error in benchmark

john-okeefe 48725544f5 refactor: use errors.Is()/errors.AsType() for error comparison and rename shadowed variables
Replace direct error equality checks (err == pgx.ErrNoRows, err != http.ErrServerClosed)
with the idiomatic errors.Is() function throughout handlers, services, middleware, and app
startup. This correctly handles wrapped error chains.

Also replace a raw type assertion (*HTTPError) with errors.AsType[*HTTPError]() in the
error handler middleware for consistency.

Additionally, rename shadowed variables for clarity:
- sidecar.go: config -> sidecarConfig, systemConfig (shadowed package-level vars)
- media_scanner.go: uuid -> uuidString (shadowed the uuid package import)

john-okeefe 4ea4393344 refactor(tests): clean up websocket test helper and fix broadcast test
- Remove createTestMediaItem helper function and replace with createTestMediaItemID
- Update TestWebSocketProgressBroadcast to use simplified helper
- Add read deadline and initial message read in TestWebSocketUserScopedBroadcast to properly consume initial connection messages
- This reduces code duplication and improves test reliability by properly handling WebSocket connection setup

john-okeefe abb090ef64 refactor(app): migrate server lifecycle to Echo v5
- Add http.Server field to App struct for explicit server management
- Add StartServer() method to create and start HTTP server
- Replace echo.Close() with http.Server.Shutdown() in Shutdown()
- Update import from echo/v4 to echo/v5

Changes:
- New() initializes server field as nil
- StartServer() creates http.Server with Echo as handler
- Shutdown() uses http.Server.Shutdown() with context timeout
- Removed deprecated echo.Close() call (v5 API change)

This provides better control over server lifecycle and graceful shutdown.

john-okeefe 4d0d86838a refactor(core): remove scheduler and simplify app lifecycle
- Delete scheduler.go and scheduler_test.go (no longer needed)
- Simplify App struct by removing Handler interface dependency
- Remove StartScheduler/StopScheduler from app lifecycle
- Update main.go to not pass handler to app constructor
- Remove scheduler mock from app tests, simplify test coverage

john-okeefe 2706ae52c1 refactor: remove Phase X terminology from source code comments
Remove planning document phase references from code comments:

app_test.go:
- Remove Phase 5 references from 8 test function comments

querier.go & queries.sql.go:
- Remove Phase 1, 2, 3, 4, 6 references from section headers
- Clean up week numbers (Weeks 5-6, Week 3-4, etc.)

queries.sql:
- Remove Phase 4 references from Kobo queries

kobo.go:
- Remove Phase 6 references from ContentId mapping comments

progress.go:
- Remove Phase 1 reference from route comment

media_scanner.go & media_scanner_library_type_test.go:
- Remove Phase 2 references from library type scanning comments

schema.sql:
- Remove Phase 1, 2, 3, 4, 5, 7 references from table/section comments
- Clean up: Format Detection, Progress Tracking, Device Registry,
  Sync Queue, Conflict Resolution, Reading History, Indexes, etc.

test_helpers.go:
- Remove Phase 6 reference from handler setup comment

These phase numbers were from internal planning documents and have no
meaning in the codebase. Removing them makes the code self-documenting.

john-okeefe 42b6fae297 chore(router): remove unused CollectionHandler from config
Remove the CollectionHandler field from router.Config struct and its
initialization in main.go. This field was never used - collections are
registered directly in handlers.SetupRoutes() where a CollectionHandler
is created locally.

Changes:
- Remove CollectionHandler field from internal/router/router.go Config
- Remove CollectionHandler: nil line from cmd/server/main.go

This cleans up dead code from the router refactoring. Collections
continue to work correctly as they are registered in SetupRoutes().

john-okeefe 0c24deb60b feat(app): implement application lifecycle management with graceful shutdown
Phase 5: Application Lifecycle Management

Creates internal/app package for proper lifecycle management, signal
handling, and graceful shutdown of all services.

Changes:
- Create internal/app/app.go with App lifecycle manager
  - Handles SIGINT, SIGTERM, SIGQUIT signals
  - Graceful shutdown with 30-second timeout
  - Manages HTTP server shutdown
  - Manages scheduler start/stop
- Update cmd/server/main.go to use app lifecycle manager
  - Replace defer-based cleanup with proper signal handling
  - Server starts in background goroutine
  - Blocks on app.Start() until shutdown signal
  - Clean shutdown of all services

Benefits:
- Proper signal handling (Ctrl+C, kill, docker stop)
- Graceful shutdown prevents data corruption
- No more os.Exit(1) bypassing defer cleanup
- All services stopped in correct order
- Server stops accepting new connections first
- Then scheduler and background services stopped

Technical details:
- Uses sync.Mutex for shutdown safety
- Context with timeout for shutdown operations
- Channel-based coordination for shutdown completion
- Logs all lifecycle events for debugging

Fixes issue where e.Logger.Fatal() would call os.Exit(1)
immediately, skipping defer cleanup and causing unclean shutdown.
